KMC Guided Care

KMC Guided Care is a comprehensive care program designed to support coordinated post-acute medical care for older adults (65+) who have been discharged from emergency departments.

This program addresses the critical gap in follow-up care to reduce hospital readmissions and alleviate the challenges patients face transitioning from the emergency department to outpatient care.

Initial Appointment Setup

A Geriatric Nurse Navigator meets with the patient in the Emergency Room to conduct a personalized needs assessment. If the patient consents to participate in the program, their first video appointment can be scheduled before they leave the Emergency Department.

White Glove Service

Patients receive a follow-up call from a member of the KMC Guided Care team 24 hours post discharge to check in and see how their doing. During the call, the team reviews follow-up information and provides assistance with preparing for their first video visit.

During the scheduled video visit, the provider reviews the patient’s health history, assesses their needs, and develops a personalized care plan focused on follow-up care, early intervention, medication management, and resource coordination.
